About School & Sports Physicals
CalvertHealth Medical Group is pleased to offer school and sports physicals to our neighbors in Southern Maryland, supporting our community youth in staying active and fit. Now is the time to get your school-age child their school or sports physical today!
The physicals are available at all of our CalvertHealth Primary Care locations during the course of regular business days.
This is a standard Sports Physical, for individuals of school age. This sports physical does not replace a full, annual physical. Services performed include: height, weight, blood pressure, vision screening, and physical exam. A review of the patient’s medical history will also be done.
If the patient has been seen in the past 12 months, the provider will fill out the sports physical paperwork based on information from that visit. Patients can drop off the forms and pick them up in roughly one week. There is no form fee. If the patient is due for a physical, the provider will fill out the forms at the visit.
Maryland Athletic Association requirement
If a student will be using our sports physical to fulfill the Maryland Athletic Association requirement for participation in Tri-County athletics including with Calvert County Public Schools athletic programs and the Southern Maryland Athletic Conference (SMAC), the student’s parent or guardian must fill out page 1 & 2 of the
physical form.
College requirement
If a student will be using our sports/school physical to fulfill a university requirement, the student must bring their school’s physical form.

Sports/School Physical appointment reminders:
- Dress lightly in loose shorts and t-shirt
- Patients under 18: MUST have parent/guardian accompany to visit
- Bring school physical form
- Student Medical History portion of form MUST be completed prior to visit
- Form MUST have parent/guardian areas completed and signed
- Bring prior shots record
- Bring photo ID and insurance card
